- Provide accessible and clear information about diagnosis, treatment options and support services
- Ensure patients have a named clinical contact for continuity of care
- Provide holistic Cancer Care Reviews (CCR) at key stages, including post-diagnosis and treatment completion
- Engage with patients and carers in shared decision-making recognising individual needs and preferences
- Link patients with local voluntary and community services who can provide additional support
