New Patients

We have an open list and welcome requests for registration from patients no matter their location. 

If you work in Cricklewood but live elsewhere and will find it easier to see a doctor during our opening hours you are welcome to register as a patient, however, we are only able to perform home visits for those patients inside of our practice area.

Register With Us

To register with us, please check you live within our catchment area; you can do this using the map below.

Once you have ensured you live within our boundary, please complete the online ‘New Patient Registration’ form.


Registration forms are also available from Reception at the Practice; you can visit us to collect one, or call us and we will provide you with one.

If you are coming to the practice, to save time you can download a registration form. In addition, please can you complete our practice registration questionnaire.

This registration form is the NHS standard GMS 1 form that we are obliged to use and which will allow us to locate your previous medical history. Whilst the questionnaire provides us with additional information (not requested on the GMS 1 form) that is essential to your care. 

We accept these forms via email but only if proof of identification is also attached. We will also accept if the completed form is handed to us in person with proof of identification.

  • If you enter an email address, we will assume implied consent to pass it on to our patient participation group. If you do not want us to pass your details on, please let us know.

Temporary Patient Registrations

You can register temporarily with a GP surgery while away from home for work, study or on holiday. You’ll remain registered with your permanent GP surgery.

Details of any treatment you have will be passed  to your permanent GP surgery. They will add the information to your medical records.

Overseas Visitors

Those who visit the UK from overseas must pay for their care when they are in England.

In 2015 changes were made to how the NHS charges overseas visitors for healthcare so the NHS does not lose out on income for the services these patients have received.

Find Your NHS Number

Sometimes when filling in online forms or speaking with our staff, you may be asked for your NHS number.

This online tool will help you find your number if you do not know it.

Please Note

You do not need to know your NHS number to use NHS services, but it can be useful to have it.

Other ways to get your NHS number

If you cannot get your NHS number online you can:

  • find it on any letter from the NHS like a prescription or appointment letter
  • call your GP surgery and ask them for your number
